At Country Roads Physical Therapy, we pride ourselves on providing expert therapeutic care in a familiar, hometown way. Located in Fairmont, Buckhannon, Wheeling, and Morgantown, we’re as committed as ever to service our clients throughout all regions of West Virginia.
Our Heritage
Founded in 2006 by Derek Falkenstein, PT, DPT, and Michael Barkley, DPT, our goal hasn’t wavered. We exist to provide the highest quality outpatient physical therapy and rehabilitation services in the mid-Atlantic region.
With an early desire to serve the good people of West Virginia, Derek and Michael opened their first location on the West Side of Fairmont; the East Side location opened for business in 2009. From there they began to expand their vision to include Buckhannon (2011), Wheeling (2013), and Morgantown (2016). Most recently, Country Roads Physical Therapy opened its third location in Marion County in White Hall.

Focused on the health and wellness of our patients, we offer a variety of therapeutic programs and services at each of our facilities, including Manual Therapy, Functional Dry Needling ASTYM, Chronic Pain Therapy, and more.

Our Team
Our Physical Therapists have been treating patients for well over a decade, and with more than 30 Physical Therapists, Physical Therapy Assistants, and Rehab Technicians across each of our six locations, we are strategically positioned to ensure our clientele receives the therapeutic treatment they need.
Country Roads Physical Therapy ensures that each staff member has received the highest level of training necessary to perform excellent service. Everyone on our staff brings something a little different to the table, but the one thing that unites us is that each patient receives the same quality treatment from the same trusted brand.
Our Vision
Alongside our tenured therapeutic treatment throughout West Virginia, we consider hometown community involvement a top priority. We offer regular educational sessions, training, and other events with the stated goal to encourage health, wellness, and informed physical care. Several of these events are free to the public.
Country Roads Physical Therapy is also involved in several local and state organizations. We regularly sponsor the Brothers of Cancer 5k, Upshur County Parish House, Fairmont Soup Opera, Fairmont Catholic, and more. Physical wellness isn’t our only goal. We exist to spread positivity and growth throughout the communities we serve.
